The appearance of lower eyelids can often be improved without surgery. Many patients have hollowing near their inferior orbital rim (the bones below the eye). Restylane or Juvederm injections can improve this hollowing and reduce the appearance of under eye bags.
Facial lines and wrinkles are often minimized with flash photography. In order to best represent real world conditions of indoor or outdoor overhead lighting it is important to look at photos taken with an overhead light, without a camera flash. Overhead lighting tends to highlight shadows and dark circles under the eyes. This type of photo may be slightly blurred, but is a better representation of what most people look like in real life. The appearance of the upper eyelid is less susceptible to changes in lighting, so flash photos are usually adequate to judge appearances before and after surgery.

These photos show a woman in her 20s with severe dark circles under her eyes. The dark circles are caused by loss of volume in the lower eyelids and upper cheeks. The patient was treated with Restylane injections into her lower eyelids and Restylane Lyft injections into the upper cheeks to produce a dramatic improvement in under eye darkness. Treating both the lower eyelids and upper cheeks is often needed to optimize results. This patient also had a small amount of Restylane Silk in her lips for a subtle enhancement of this area.

These photos show a woman in her 30s with lower eyelid hollowing causing "bags" under the eyes. The hollowing causes her to have a tired appearance even when she gets a full night sleep. Restylane injections to the lower eyelids help reduce the hollowing and promote a smooth contour from the eyelid margin to the cheek.
As mentioned above, the goal of lower eyelid rejuvenation for most patients is to create a smooth contour from the lower eyelid to the cheek without a harsh transition zone. The two factors which prevent this smooth contour are hollowing near the orbital rim (bone under the eye) and presence of excess fatty tissue. If the main problem is hollowing at the orbital rim, as in this patient, Restylane injections are the treatment of choice. Patients with a lot of excess fatty tissue need fat removal and are best treated with lower blepharoplasty surgery. Many patients with mild amounts of fat herniation desire non-surgical treatment and can see a significant improvement from Restylane injections as well.
Patient 3 - Before and after non-surgical blepharoplasty with Filler Injections
These photos show a woman in her 20s before and after filler injections to the lower eyelids and upper cheeks. Before treatment, there is a significant amount of puffiness in the lower eyelids (lower eyelid bags). This condition is usually best treated with lower blepharoplasty surgery to remove the excess fatty tissue from the lower eyelids. In patients who prefer to avoid surgery, the bags can be camouflaged by injecting fillers under the bags and into the upper cheeks. Camouflaging lower eyelid bags instead of removing them usually works best in younger patients in their 20s and 30s, but older patients can sometimes benefit as well.

These photos show before and after photos taken without a camera flash to highlight shadows from overhead lighting. Restylane was injected under the eyes and Restylane Lyft was injected into the upper cheeks. After treatment, the prominence of the under eye bags is greatly diminished.

In addition to reducing the appearance of the eye bags, the treatment results in youthful elevation of the cheeks. This is best appreciated in the photos taken with a camera flash.

These photos show another patient with a harsh transition between the lower eyelid and the cheek from hollowing along the orbital rim. Filler injections help smooth this transition, eliminating the look of being "tired".
These photos show patients who have had injections across the lower eyelid. Some patients need injections only at the portion of the lower eyelid closest to the nose, often referred to as the tear trough. Follow the link in the previous sentence to see before and after photos of this procedure.
Restylane and Juvederm are FDA approved to treat smile lines (wrinkles in the nasolabial fold). Use of Restylane and Juvederm in other areas of the face is considered an "off-label" use of the medication.
